For the convenience of our clients, we provide several business and customs brokerage forms. All of these forms are in Microsoft Word (.doc) or Adobe PDF format. You can complete them using an editing program or print them.

For businesses, we offer a bill of lading, application for credit, and cargo insurance coverage request forms. We offer the same forms in regards to customs brokerage, in addition to several other relevant documents. To make things easy, we list everything you need to ship your freight across the country and over borders. These forms include FDA completion guidelines, prior notice forms, manufacturer’s affidavits, and shipper’s letter of instruction guidelines.

C-TPAT

Customs-Trade Partnership Against Terrorism

Speed Global Services is a validated participant in the Customs-Trade Partnership Against Terrorism (C-TPAT) Program. The Custom-Trade Partnership Against Terrorism is a joint government and business initiative to build cooperative relationships that strengthen the overall supply chain and border security. C-TPAT recognizes that U.S. Customs and Border Protection (CBP) can provide the highest level of security only through close cooperation with the ultimate owners of the supply chain importers, carriers, brokers, warehouse operators, and manufacturers.

Speed Global Services is asking businesses to ensure the integrity of their security practices and communicate their security guidelines to their business partners within the supply chain.

If you are already enrolled in the C-TPAT program, we ask you Log-In to your C-TPAT Portal and request to be monitored by Royal Customs Brokers, Inc. DBA Speed Global Services and Speed Motor Express of WNY, Inc. DBA Speed Global Services if you are utilizing both our Customs Brokerage and our trucking Warehouse services.

Also, complete the first page of the Supply Chain Questionnaire-Business Partners. The remaining questions do not need to be completed if you are enrolled in the C-TPAT program.

As a C-TPAT Partner and Participant, Speed Global Services is required to screen security procedures and practices being utilized by all businesses in our supply chain.

Please see the C-TPAT Participant & Security questionnaire in the link under important documents. Please complete the entire questionnaire if you are not enrolled in the program to the best of your ability and return it to us immediately.
